Registration Summer 2008 / RECREATIONAL LEAGUE
The $50.00 late registration fee is in effect for
junior level.
Senior starting later it will be no late fee until further notice.
All late registrations will be accepted.
Walk-in registration only. No
registration by mail
Soccer Office during office hours (please
check Website for office hours)
ABSOLUTELY NO registrations will be processed without a:
a) PHOTOCOPY of your child’s Medicare card
b) Proof of residence (either Bell, Hydro, or similar bill,
report card, driver license will do) with the player’s
family name.
c) Full payment of fees
d) Completed registration and FQSF bordereau form both SIGNED
and DATED by the player(14 &+) or a PARENT
e) 3 recent passport photos of the player
Please note that each player MUST have separate forms.
First & last name of player must be the same as on the
medicare information
Please read carefully all the registration form information
The House League Season will begin Monday, June 2, 2007 unless
otherwise stated by the City of Montreal, CDN/NDG Burroughs.
